FALL RIVER, Mass. ( WPRI) – Investigators have identified two potential causes of the deadly fire at an assisted living facility in Fall River, Massachusetts. But since they can’t pinpoint the exact origin, the cause of the fire has officially been ruled “undetermined.”

The Bristol County District Attorney’s Office released the previously withheld name of one of the 10 victims killed in the July 13 fire.

Smoking or an oxygen machine issue may have sparked the deadly fire at Gabriel House Assisted Living Facility in Fall River, Massachusetts, a state fire marshal said.
